This form includes questions regarding what you want to accomplish through your participation in the Main Street Leadership Collaborative™ Course. Payment may be made by check or credit/debit card (through PayPal). Tuition payments may be made as follows: $450 If registered by 04/18/19 — $375 (17% savings if registered by 04/01/19) — $100 deposit to “reserve your seat in the course”. Full payment must be made by April 18, 2019 for you to attend the course.

A payment must be made with this registration. To participate
in the Main St Leadership Course, complete payment must be made
by April 18, 2019.

your payment, click the "Return to Merchant" button to return to the
Main Street Leadership website.

• $100 -- MSLC Minimum Deposit
• $375 -- MSLC Early Bird Tuition (By 04/01/19)
• $450 -- MSLC Full Tuition (After 04/18/19)
